If you have feedback about your experience at York Street General Practice, we welcome you to share it with us. You can provide feedback in person at reception, by phone, or in writing via email. All feedback is treated respectfully and confidentially and helps us improve our services.
If you have a concern about your privacy or care, please speak with our Practice Manager in the first instance. We aim to respond promptly and work with you to resolve any issues. If you are not satisfied with the outcome, you may escalate your complaint to the appropriate Australian or NSW health complaints authority.
York Street General Practice is closed on public holidays. If you require urgent medical assistance during this time, you may choose to be redirected to the on-call doctor (Dr Richard Oak or Dr Stuart Baines). Please note that a fee applies for after-hours consultations.
In the case of a medical emergency, please dial 000 or attend your nearest emergency department. For non-urgent matters, we encourage you to contact the practice during normal business hours.
Home visits may be available for existing patients of York Street General Practice whose condition prevents them from attending the practice. These are offered at the discretion of the treating doctor and are limited to patients within the local area.

If you would like your medical records transferred from another practice to York Street General Practice, please complete a transfer request form available from our reception team.
Please confirm with the practice directly if any fees apply.
If you are transferring your records from York Street General Practice to another practice, please complete and sign a records transfer form at your new practice to authorise the release of your health information. All requests must be made in writing and signed by the patient. Requests are processed in accordance with privacy legislation. No fees apply.
